Instructions to Obtain an Affidavit of Residence
 
Please note: Affidavits of Residence must be obtained through the Anderson 5 District Office, located at 400 Pearman Dairy Rd. Anderson, SC 29625. 
 
Steps:
  1. The parent/guardian of the student and the individual with whom they live must go to the District Office together with their picture IDs.
  2. Homeowner/Lessee must provide two currrent (within the last month) proofs of residence
    • Valid proofs include: powerl bill, water bill, landline phone bill, cable bill, natural gas bill, or other utility connected to the home, rental agreement, closing statement or mortgage statement. 
  3. Affidavit of Residence paperwork will be filled out, signed by both parties, and notarized.
  4. Notify the school that an Affidavit of Residence was received. Please note that a copy will need to be provided to the school.
